Position Overview
Empowering Success Now is seeking an experienced, compassionate, organized, and community-driven Case Manager to oversee and support client caseloads across our programs.
The Case Manager will serve as a central point of connection between our clients and ESN's different departments. This individual will help ensure clients are successfully connected to the services they need, remain engaged in their programs, receive appropriate follow-up, and do not fall through the cracks.
The Case Manager will conduct client intakes, assess needs, coordinate services, schedule appointments, follow up with clients, maintain case records, track outcomes, advocate for clients, and collaborate with ESN program staff to ensure clients receive appropriate support.
Case Management Responsibilities
- Manage and monitor client caseloads across ESN's Mental Wellness, Job Placement Assistance, Community Resources, and Skills Training programs.
- Serve as a primary point of contact for clients navigating one or multiple ESN services.
- Conduct client intakes and gather information regarding the client's needs, goals, barriers, and requested services.
- Help determine which ESN program or services may best meet each client's needs.
- Connect clients with the appropriate ESN department, staff member, program, appointment, training, or resource.
- Schedule and coordinate client appointments and program bookings as needed.
- Maintain regular communication with clients through phone calls, email, text, appointments, and other approved communication methods.
- Follow up with clients to ensure they received the services or resources they were connected to.
- Monitor client participation and progress throughout ESN programs.
- Re-engage clients who miss appointments, become inactive, or stop participating before completing services.
- Motivate and encourage clients to continue utilizing services that can help them achieve their goals.
- Identify additional needs or barriers that may arise during a client's participation and connect them with appropriate resources.
- Advocate for clients when they need assistance navigating services, organizations, programs, or community resources.
- Assist individual departments with their caseloads when additional support is needed.
- Conduct periodic client check-ins to determine whether additional services are needed.
- Follow up with clients after services are completed to determine outcomes and whether additional assistance is necessary.
- Help clients transition out of services appropriately once their needs have been met.
- Maintain professional boundaries and protect confidential client information.
Documentation, Data & Grant Support
Accurate documentation is an important part of this position because ESN uses program data to understand its impact and meet reporting requirements.
The Case Manager will:
- Maintain accurate, organized, and timely client files and case notes.
- Document client contacts, appointments, referrals, services provided, participation, progress, and outcomes.
- Maintain updated client rosters and caseload information across departments.
- Track client enrollment, participation, completion, referrals, and other program outcomes.
- Assist with collecting accurate program numbers and statistics needed for grant reporting.
- Help identify success stories, program outcomes, service numbers, and other information needed to demonstrate program impact.
- Work with program staff to ensure client information and program outcomes are being documented consistently.
- Assist with preparing internal reports and information requested by leadership.
- Maintain confidentiality and follow organizational policies regarding client information and records.
Client Advocacy & Resource Navigation
- Build trusting and professional relationships with clients from diverse backgrounds.
- Listen to clients' needs and help identify practical next steps.
- Advocate for clients while encouraging clients to become confident advocates for themselves.
- Research community resources when ESN cannot directly provide a requested service.
- Connect clients with appropriate outside organizations and service providers.
- Help clients understand available programs, requirements, appointments, and next steps.
- Maintain knowledge of community resources available throughout Fontana and surrounding communities.
- Work collaboratively with ESN staff to provide coordinated support instead of treating each program as a separate service.
Community Outreach
The Case Manager & Partnership Coordinator will also represent Empowering Success Now throughout the community.
Responsibilities may include:
- Attend community outreach events, resource fairs, job fairs, networking events, workshops, and community meetings.
- Educate community members about ESN's programs and available services.
- Help identify individuals and families who may benefit from ESN services.
- Conduct outreach and assist potential clients with connecting to programs.
- Professionally represent ESN's mission and services when interacting with community members, businesses, organizations, and community leaders.
- Develop relationships with other organizations serving populations that may benefit from ESN programs.
- Help increase awareness, enrollment, participation, and successful completion across ESN programs.
Partnership Coordination
Partnership development is a secondary but important responsibility of this position.
The Case Manager & Partnership Coordinator will:
- Identify potential partnerships that can strengthen any of ESN's four primary service areas.
- Develop and maintain relationships with businesses, employers, nonprofits, educational institutions, community organizations, resource providers, and other agencies.
- Identify employers willing to work with ESN's Job Placement Assistance Program and consider qualified candidates for interviews and employment opportunities.
- Develop partnerships that can provide additional training, workshops, resources, referrals, or opportunities for Skills Training participants.
- Identify organizations that can provide resources or accept referrals from ESN clients.
- Introduce organizations to ESN's mission, programs, and opportunities for collaboration.
- Maintain communication with existing partners rather than allowing partnerships to become inactive after the initial connection.
- Attend networking and community events to develop new relationships and strengthen existing ones.
- Work with ESN leadership and program staff to identify partnership needs within each department.
- Maintain organized records of partnerships, contacts, referrals, and collaborative opportunities.

This position requires someone who genuinely enjoys working with people and understands that successful case management requires consistent follow-through.
We are looking for someone who will not simply give a client a phone number or referral and consider the case complete. We want someone who will follow up, ask whether the client received assistance, identify what happened if they did not, and help determine the next step.

Qualifications
Required:
- Previous professional experience in case management, social services, community services, client services, nonprofit programs, human services, outreach, resource navigation, or a closely related field.
- Demonstrated experience managing or supporting a client caseload.
- Strong communication and interpersonal skills.
- Strong organizational and time-management skills.
- Experience conducting client follow-ups and maintaining documentation.
- Comfortable using computers, email, databases, spreadsheets, calendars, and other client-management tools.
- Ability to handle confidential and sensitive client information professionally.
- Ability to work full-time from the Fontana office.
- Reliable transportation for approved community outreach, meetings, networking events, and partnership activities.
Preferred but Not Required:
- Associate's or bachelor's degree in Social Work, Human Services, Psychology, Sociology, Business, Communications, or a related field.
- Experience working for a nonprofit or community-based organization.
- Experience with community outreach or partnership development.
- Experience collecting program data or assisting with grant reporting.
- Knowledge of local community resources.
- Bilingual English/Spanish skills are a plus.
Relevant professional experience may be considered in place of a college degree.
