We are seeking a licensed Physical Therapist who is passionate about delivering high-quality care and building meaningful relationships with patients and their families.
Responsibilities
- Conduct comprehensive physical therapy evaluations in the home setting
- Develop individualized treatment plans based on patient needs and goals
- Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, and functional independence
- Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety strategies
- Monitor patient progress and modify treatment plans as appropriate
- Complete accurate and timely clinical documentation
- Collaborate with physicians, nurses, case managers, and interdisciplinary team members to ensure continuity of care
- Maintain compliance with all state, federal, and agency regulations
Qualifications
Required
- Active California Physical Therapist (PT) License
- Graduate of an accredited Physical Therapy program
- Current CPR certification
- Valid driver's license and reliable transportation
- Strong communication and interpersonal skills
Preferred
- Home Health experience
- Experience working with geriatric populations
- Familiarity with electronic medical record (EMR) systems
