We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Aristos Rehabilitation and Nursing Center. As a LPN, you will play a vital role in providing high-quality patient care to our residents and exceeding their expectations.
About the Role:
As a LPN, you will be responsible for:
Providing direct patient care to residents, including administering medications, wound care, and vital sign monitoring
Assisting with daily living activities, such as bathing, dressing, and grooming
Developing and implementing individualized care plans in collaboration with residents, families, and other healthcare professionals
Maintaining accurate and up-to-date records of patient care and treatment
Participating in interprofessional team meetings to discuss patient care and progress
Responsibilities:
Patient Care: Provide compassionate and skilled care to residents, including administering medications, wound care, and vital sign monitoring
Teamwork: Collaborate with healthcare professionals to develop and implement individualized care plans
Communication: Effectively communicate with residents, families, and healthcare team members regarding patient care and treatment
Documentation: Maintain accurate and up-to-date records of patient care and treatment
