One Client Home-Based Care Self-Determination Program

We are seeking a dependable, compassionate, and physically capable individual to provide one-on-one Community Living Supports (CLS) for an adult with physical and developmental disabilities through a Self-Determination program.
This is one client in one home—not a facility position and not multiple clients. You’ll work directly with one individual to promote independence, mobility, community participation, and achievement of Person-Centered Plan (PCP) goals.
Available Schedule
Part-time position with approximately 17.5–30 hours per week, depending on staffing needs, school schedules, and employee availability.
Position Includes:
Three Assigned Weekday Afternoon Shifts
Monday Evening/Overnight Shift
Additional daytime, evening, weekend, and school-break hours may become available based on staffing needs.
Important Scheduling Information
The work schedule is assigned based on client and staffing needs.
Employees do not choose their own days or customize their schedules.

Primary Responsibilities
- Implement Person-Centered Plan (PCP) goals
- Assist with community outings and community participation
- Support life skills development and independence
- Assist with standing programs using adaptive equipment
- Assist with gait training using a pacer
- Perform Hoyer lift transfers
- Assist with mobility, positioning, and therapeutic activities
- Provide personal care, hygiene assistance, toileting, and brief changes as needed, including during community outings
- Assist with meal preparation and self-feeding
- Complete required documentation
- Maintain a safe, clean, and respectful environment
Physical Requirements
This is a physically active position requiring applicants to safely perform the essential functions of the job, including:
- Frequent lifting, bending, stooping, kneeling, and repositioning
- Hoyer lift transfers
- Assisting with standing programs and gait training using adaptive equipment
- Pushing and maneuvering adaptive mobility equipment
- Providing hands-on physical assistance throughout the shift

Qualifications
- Reliable transportation
- Excellent attendance and dependability
- Comfortable providing one-on-one support in a home setting
- Ability to safely perform the physical requirements of the position
- Experience as a Caregiver, Direct Support Professional (DSP), Community Living Support (CLS) worker, CNA, Home Health Aide, or related field is preferred but not required.
Pay: $22.20 - $22.40 per hour
