Overview
We are seeking a dedicated and highly skilled Long-Term Care Registered Nurse to provide comprehensive nursing care in a long-term care facility. The ideal candidate will possess a strong background in geriatrics, memory care, or post-acute care, with the ability to deliver compassionate, patient-centered services. This role offers an opportunity to make a meaningful difference in the lives of residents by ensuring their health, safety, and well-being through expert clinical practice and collaborative care planning.
- Pay Rate: $45.50
- Shift: PM and NOC, Every other weekend required
- LPNs also encouraged to apply ($37.50/hr)
Duties
- Conduct thorough patient assessments, including vital signs, physical examinations, and evaluation of medical histories to develop individualized care plans.
- Administer medications.
- Coordinate with interdisciplinary teams.
- Document all patient interactions accurately.
- Educate residents and their families on health coaching, medication administration, and disease management to promote optimal health outcomes.
- Support infection control practices and participate in quality improvement initiatives aligned with hospital medicine standards.
Qualifications
- Valid Wisconsin RN (Registered Nurse) license
Pay: $45.50 per hour
