Licensed Social Worker
A healthcare organization is actively seeking a Licensed Social Worker to join an Adult Day Care Program, located in Staten Island.
Maintain and/or improve participants' ability to manage daily physical, emotional, and psychosocial needs.
Identify individual social and emotional needs through ongoing assessment.
Develop and maintain individualized care plans.
Participate in admission, community re-entry, and transfer planning as appropriate.
Collaborate with interdisciplinary team members to ensure comprehensive treatment of social and emotional needs.
Attend and contribute to care plan meetings and case conferences.
Licensed Social Worker (LSW) or Master of Social Work (MSW).
Previous Long-Term Care and/or Subacute experience required.
Strong communication, organizational, and interpersonal skills.
Ability to work effectively as part of a multidisciplinary team.
