Job Summary
Part Time Position: Sunday/Tuesday/Thursday
The Wound/Treatment Nurse provides specialized care for residents with wounds, ostomies, and skin integrity issues in our 160-bed skilled nursing facility. This role includes comprehensive assessments, treatment planning, prevention of pressure injuries, staff education, and full compliance with CMS/CDPH regulations.
Key Responsibilities
- Conduct wound assessments, staging, and ongoing monitoring.
- Develop, implement, and evaluate individualized wound care plans in collaboration with the interdisciplinary team.
- Perform direct treatments, dressing changes, and debridement (per scope).
- Prevent and manage pressure ulcers and skin breakdown.
- Educate residents, families, and staff on wound care and prevention.
- Serve as a clinical resource for physicians and nursing staff.
- Maintain accurate EHR documentation and support quality improvement initiatives.
- Conduct staff in-services on wound management best practices.
Qualifications
- Active California nurse license.
- Associate or BSN degree (BSN preferred).
- 1–2 years nursing experience; SNF or wound care experience preferred.
- BLS/CPR required; Wound Care Certification (CWCN/WCC) preferred.
- Strong knowledge of wound management, infection control, and SNF regulations.
- Excellent communication skills; bilingual (English/Spanish) a plus.
Physical Requirements
- Ability to lift/position residents, stand/walk for extended periods, and perform hands-on care.
- Must pass background check and health screenings.
