Heartsong Hospice is seeking an experienced and motivated Patient Care Advocate to join our growing hospice team.
We are looking for a relationship-driven professional with a strong healthcare sales background and an established referral network who is passionate about helping patients and families receive compassionate end-of-life care.
Position Responsibilities
As a Patient Care Advocate, you will:
- Develop and maintain strong referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and discharge planners
- Perform territory market analysis and execute quarterly and annual business development plans
- Consistently generate new referral sources and business opportunities
- Collaborate closely with clinical and leadership teams to support census growth initiatives
- Participate in the referral and admissions process.
- Represent Heartsong Hospice professionally within the community and healthcare market
Qualifications
Preferred Experience:
- 3–5 years of hospice, home health, or healthcare sales and marketing experience
- Existing referral base within the Columbia and surrounding counties
- Strong understanding of post-acute healthcare services and discharge planning processes
Additional Requirements:
- Excellent communication, organizational, and relationship-building skills
- Ability to work independently and meet performance goals
- Valid driver’s license, reliable transportation, and proof of auto insurance required
