We are seeking a dedicated and compassionate personal caregiver to join our Personal Care Team in a part-time/PRN capacity. As a personal caregiver, you will play a crucial role in providing high-quality, personal, patient care and ensuring the well-being of our patients and their families. If you are passionate about making a difference in the lives of others and possess excellent caregiving skills, we encourage you to apply.
Responsibilities:
- Perform personal cares for patients where they call home
- Follow established care plans created by the patient and nursing team
- Monitor and evaluate patients' responses as you assist them with ADL's
- Provide education and support to patients and their families regarding their healthcare needs
- Document all patient interactions, assessments, and interventions accurately and timely
- Adhere to infection control protocols and maintain a safe environment for patients
Experience:
- Proficiency in safety of transferring patients from various positions
- Experience working in personal care, home health, hospice, skilled nursing facility, assisted living facility is a plus
Qualifications:
- Excellent communication and interpersonal skills
- Strong critical thinking and problem-solving abilities
