Northern Manhattan Perinatal Partnership (NMPP) a maternal and child health agency located in Central Harlem seeks a Family Resources and Support Specialist (FRSS) for its Healthy Families Central Harlem Program. The goal of NMPP is to assist women of childbearing age have healthy birth outcomes and to assist families with challenging issues.
The goal of the Healthy Families Program is to prevent child abuse and neglect through the promotion of child health and development, positive parent-child interaction and family self sufficiency. Intensive home visiting services are provided to families that are pregnant or have a baby under three months.
The Family Resource and Support Specialist is expected to
* Follow Healthy Families of America Best Practice Standards, Healthy Families of New York, Healthy Families Central Harlem, and NMPP's polices and ethical standards for human services professionals.
* Establish trusting relationships with families from diverse backgrounds. Meet with caregivers prenatally or post-birth up to the age of five in the clients homes..
* Utilize preventive and early intervention using strength based and evidence based curriculums, parent lead child activities and promote healthy development and healthy well being.
* Promote family health and safety by providing education around education around safe sleep, health hazards, lead, immunization and well baby visits
* Collaborate with participants to develop and revise family goal plans
* Liaise with referral agencies and track participant's engagement.
*Create and maintain accurate and timely case records.
* Monitor participant's progress to recommend case closure based on the achieved goals, non participation.
* Assist with the recruitment of clients such as, street outreach, parenting workshops, community events
* Work in a team based setting, participate in team meetings, case conferences
* Complete annual training related to child abuse, neglect and neglect, diversity
* Participate in ongoing professional development and training
* Accommodate families schedule and support NMPP's events as needed.
QUALIFICATIONS
* Associates or Bachelor's degree in Social Work, Social Sciences, Psychology, Human Services or any closely related field. Two years work experience in human services.
* Strong writing skills
* Ability to make home visits
* Ability to work with a team and independently
* Functional knowledge of operating a computer, and software such as Microsoft 365, Google Chrome.
* Experience in the social services field
* Good engagement skills
* Perform other duties as required
* Bilingual English/Spanish
