Apex Health Solutions LLC is a licensed home care agency serving clients throughout Bergen County and surrounding areas in New Jersey. We are committed to delivering compassionate, high-quality, patient-centered care in the comfort of our clients' homes. We are currently seeking dedicated LPNs to join our growing team and provide skilled nursing services to private-pay clients. The ideal candidate is compassionate, reliable, organized, and passionate about helping individuals maintain their independence and quality of life at home.
Responsibilities
- Provide skilled nursing care to clients in their homes in accordance with the Plan of Care.
- Monitor and assess clients' physical condition and report changes to the Registered Nurse (RN), Director of Nursing, physician, and family as appropriate.
- Administer medications and treatments as prescribed by the physician and within the LPN scope of practice.
- Perform routine nursing procedures including wound care, dressing changes, catheter care, ostomy care, and vital sign monitoring.
- Observe, document, and report patient responses to treatments and interventions.
- Educate clients and family members regarding disease management, medications, safety precautions, and home care needs.
- Maintain accurate and timely clinical documentation in accordance with agency policies and state regulations.
- Collaborate with the RN Supervisor, physicians, therapists, Home Health Aides (HHAs), and other members of the healthcare team.
- Assist in developing and implementing individualized care plans.
- Promote client independence, dignity, comfort, and quality of life.
- Ensure compliance with infection control, safety, HIPAA, and agency policies.
Qualifications:
- Current and active New Jersey Licensed Practical Nurse (LPN) license in good standing.
- Graduate of an accredited Practical Nursing program.
- Minimum one (1) year of nursing experience preferred; home care experience is a plus.
- Current CPR/BLS certification.
- Valid driver's license, reliable transportation, and auto insurance.
- Ability to travel to clients' homes throughout Bergen County and surrounding areas.
- Knowledge of nursing practices, medication administration, wound care, and patient assessment.
- Ability to accurately document patient care using electronic or paper records.
- Strong communication, organizational, and time-management skills.
- Ability to work independently and collaboratively with physicians, RNs, HHAs, patients, and family members.
- Ability to maintain patient confidentiality and comply with HIPAA regulations.
- Must pass background check, reference verification, and health screening requirements.
Pay: $45.00 - $55.00 per hour
