Position Overview
The Resident Care Manager (RCM) plays a key leadership role in ensuring the delivery of the highest quality, resident-centered care. The RCM promotes resident independence, dignity, and well-being through clinical excellence, effective team leadership, and interdisciplinary collaboration.
This position oversees a 14-bed sub-acute rehabilitation unit, providing hands-on guidance to nursing staff and ensuring regulatory compliance and clinical best practices.
Key Responsibilities

Maintain 24-hour accountability for unit operations and participate in the on-call rotation

Assess, monitor, and coordinate individualized care plans for residents.

Supervise, mentor, and evaluate nursing staff performance.

Ensure compliance with all state and federal regulations and uphold resident rights.

Collaborate with physicians, interdisciplinary teams, and families to optimize care outcomes.

Participate in quality improvement, infection control, and staff development initiatives.

Collaborates with RNAC/MDS coordinator to establish individual care plans based on the Care Area Assessment triggers from the Minimum Data Set ( MDS)

Qualifications:

Current New Jersey RN License (required).

Bachelor of Science in Nursing (BSN) preferred.

2–5 years of experience in long-term care, sub-acute, or acute care.

Proven leadership experience in nurse management, including staff development and performance management.

Strong understanding of state and federal long-term care regulations and compliance practices.

CPR and AED certification required.

Experience with MDS preferred.
