Position Overview
A leading healthcare system in Cleveland is seeking experienced Registered Nurses to join its Community Living Center team. This position provides comprehensive nursing care for adult patients requiring long-term care, rehabilitation services, skilled nursing support, chronic disease management, and complex medical care.
The RN will work collaboratively with an interdisciplinary healthcare team to provide high-quality, patient-centered care while promoting safety, dignity, and quality of life for residents.
Key Responsibilities
- Perform comprehensive patient assessments and ongoing monitoring
- Administer medications and evaluate treatment effectiveness
- Provide IV therapy and fluid replacement
- Monitor telemetry and cardiac rhythms when indicated
- Coordinate admissions, discharges, and patient transfers
- Collaborate with physicians, therapists, case managers, and ancillary staff
- Delegate and supervise nursing assistants and support personnel
- Complete accurate electronic medical record documentation
- Respond to medical emergencies and changes in patient condition
- Perform central line care and maintenance
- Manage anticoagulation therapies including heparin drips
- Provide chest tube and drainage tube management
- Perform Foley catheter and ostomy care
- Deliver wound care and wound vacuum therapy management
- Assess and manage acute and chronic pain
- Support pre-operative and post-operative patient care
- Administer blood products and monitor for adverse reactions
- Provide oncology-related symptom management
- Support rehabilitation and neurological recovery plans
- Monitor for sepsis, respiratory compromise, cardiac events, and other acute conditions
- Participate in quality improvement initiatives and departmental meetings
- Maintain compliance with all clinical, regulatory, and patient safety standards
Qualifications
- Graduate of an ACEN or CCNE accredited School of Nursing
- Active Ohio RN License or active Compact RN License
- Current American Heart Association CPR Certification
- Current Basic Life Support (BLS) Certification
- Minimum two (2) years of direct patient care experience within the last three (3) years
- Strong clinical assessment and critical thinking skills
- Experience with telemetry monitoring and cardiac rhythm interpretation preferred
- Proficiency with electronic health record documentation
- Ability to communicate effectively in English
- Ability to meet all credentialing and onboarding requirements
- Flexibility to work days, evenings, nights, weekends, and holidays as needed
