Duties
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to determine individual needs.
- Develop and implement tailored care plans that incorporate discharge planning, post-acute care coordination, and home health services.
- Manage clinical case management activities by monitoring patient progress, adjusting interventions, and ensuring adherence to treatment protocols.
- Collaborate closely with healthcare professionals across hospital medicine, behavioral health, infectious disease care, and primary care to optimize patient outcomes.
- Utilize the Electronic Medical Record (EMR) system to document medical records accurately while maintaining strict compliance with HIPAA regulations.
- Facilitate communication with patients and their families to provide education on medical terminology, medication management, and health regulation policies.
- Oversee utilization management processes by evaluating diagnostic evaluations and coordinating appropriate services for inpatient, outpatient, or hospice care settings.
Requirements
- Valid nursing license with current registration; experience as a registered nurse in acute care settings such as ICU, Emergency Room, or Level I Trauma Center preferred.
- Demonstrated critical care experience including Level II trauma center or Level I Trauma Center experience is highly desirable.
- Proven background in hospital medicine, medical-surgical units, or home health nursing with familiarity in managing complex cases involving geriatrics or pediatrics.
- Strong knowledge of physiology, medical terminology, infection control practices, and health regulation policies.
- Experience with EMR/EHR systems for documentation and case tracking; proficiency in managing electronic health records is essential.
- Excellent patient service skills with the ability to communicate effectively across diverse populations; experience in client communication and discharge planning is advantageous.
- Ability to work collaboratively within multidisciplinary teams while managing multiple priorities efficiently in inpatient or outpatient environments.

Pay: $1,540.00 - $1,692.00 per week
