DAWN Center for Independent Living (DAWNcil) is a grassroots consumer-driven and consumer-controlled nonprofit organization promoting self-direction in order to create, expand, and implement equal-access opportunities for people with disabilities.
Our Early Intervention Unit at DAWN Center for Independent Living is growing! To fill this position, DAWNcil seeks an enthusiastic and self-driven team member to coordinate services and provide care management to families enrolled in New Jersey’s Early Intervention System (NJEIS). A successful candidate must be able to multitask and quickly transition between job responsibilities and be flexible to unanticipated changes. Job requires travel within Morris/Sussex Counties and candidates must have a dependable means of transportation.
The mission of NJEIS is to enhance the capacity of families to meet the developmental and health-related needs of children, birth through age three, who have delays or disabilities. The Service Coordinator works with the family and the team of EI practitioners to create an Individualized Family Service Plan, set goals for the child, assign designated services, and monitor the progress of the child.
Qualifications:
- Must possess a Bachelor's degree in a social services field (social work, psychology, etc.).
- At least one year of documented professional experience working with individuals/children with disabilities post-graduate.
- Knowledge of and support for the Independent Living Philosophy.
- Ability to successfully relate to and communicate with people with disabilities, their families, other organizations, agencies, volunteers, and consumers.
- Flexible, resilient, and possess outstanding time management capabilities and the ability to organize work effectively, establish priorities and express self professionally.
- Strong team player who works effectively with all levels within an organization, takes initiative, practices strong self-leadership, and has strong follow-up and follow-through and quality assurance skills.
- Strong critical, strategic, and creative thinking capabilities necessary to assist consumers and their families create high-quality, individualized service plans to include identifying potential services and agencies they can benefit from.
- Accepts developmental feedback and enjoys continuous learning and development.
- Has the ability to work with minimal supervision and meet deadlines.
- Strong and effective verbal and written communication skills.
- Experience using Microsoft Office suite and ability to use other technology as per position requirements efficiently.
- Possess a valid driver's license and proof of auto insurance coverage, limits of at least $100,000 / $300,000.
Key Responsibilities:
- Work collaboratively with families and providers to write effective and unique Individual Family Service Plans (IFSP).
- Provide ongoing monitoring and management of the IFSP and provide assistance with managing their services and any concerns or challenges identified.
- Schedule and facilitate all needed meetings for the management of the child's services.
- Collaborate and maintain relationships with various community partners to ensure familiarity and in-depth understanding of service options.
- Maintain compliance with Early Intervention policy and procedures and adherence to established deadlines and benchmarks.
- Other tasks deemed essential to the success of the consumers and agency
