Job duties:
Support/Instruction for Persons Served
· Provides support and instruction to one or more persons served in a variety of community and site-based activities and environments. Includes responsibility for the safety of individuals served in all settings.
· Assists program participants to access community services/resources, explore educational, social, recreational and employment opportunities and make/maintain connections.
· Provides an array of services needed to achieve each person’s individual goals and objectives, including: use of assistive technology, job analysis, task analysis, assessment, systematic instruction; and ongoing support services as is necessary.
· Assists program participants to develop the tools necessary to achieve goals such as obtaining employment, accessing preferred environments, and making connections with others (i.e., resume, master application, DMV ID, etc.)
· Provides other types of support and instruction to persons served as requested.
Develop/Implement Service Plans and Write Reports
· Participates in the development and implementation of Individual Service Plans based on participant goals and objectives.
· Collects data and writes and submits reports per Napa Valley PSI and funding/regulatory agency expectations.
· Maintains records and files for persons served in accordance with Napa Valley PSI policies.
· Reports all suspected abuse in accordance with mandated reporter regulations and procedures and adheres to Napa Valley PSI expectations for reporting special incidents.
· Adheres to all related agency policies and procedures.
Develop and Maintain Effective Relationships
· Displays a professional and respectful attitude towards individuals served, coworkers, supervisors, community employers and other agency affiliates.
· Works effectively as part of the Napa Valley PSI team to accomplish the mission of the organization. This includes developing and maintaining positive relations with families, other agencies serving individuals with disabilities, funding sources/other government agencies and all entities that provide services which overlap with programs provided by Napa Valley PSI.
· In coordination with persons served, facilitates and assists individuals to develop a circle of support that builds and maintains connections with family, friends, neighbors, and community members.
· Models person centered planning philosophies in all work efforts and consistently promotes persons served to be at the center of making decisions that impact their own lives.
· Contributes to a team approach to solving issues related to persons served.
· Provides for continuous quality service improvement and promotes teamwork among co-workers and persons served.

Prerequisite Qualifications
The candidate must meet the following criteria to be considered for employment in this position:
· Minimum one (1) year experience working with individuals with disabilities preferred.
· Experience in job/task analysis, work assessment, teaching techniques or equivalent experience is desirable.
· High School diploma or equivalent preferred. Bachelor’s degree desirable.
· Prior to employment, obtain successful results from the DOJ/FBI fingerprints and background screening within 10 working days from the date of appointment.
· Prior to employment, pass the placement physical which will determine physical ability to perform the essential functions of the position which includes a physical, skin T.B. test or chest x-ray, breath alcohol test (BAT), and drug screen.
· Prior to employment, provide proof of a valid California Driver’s License, proof of active car insurance for yourself and the vehicle that you will be using during work hours, and an acceptable DMV driving record dated no later than 30 days from date of offer letter (to be approved by PSI or MIV Insurance).
· Prior to employment, provide documents to validate eligibility to legally work in the United States.
· Ability to work a flexible schedule which may include nights, weekends and holidays.
· Ability to self-direct daily work activities with little supervision.
· Experience with Windows operating system, Microsoft Office and database reporting.
· Maintain orderly work environment and perform tasks in a prescribed and safe manner.
· Ability to take direction from supervisor and respond to requests for assistance from other co-workers in a timely manner.
· Willingness to enroll in ongoing, free training to expand and develop skills.
