- Monitor and report any changes in client condition or behavior to the care team
- Collaborate with other care professionals to develop and implement personalized care plans
- Maintain accurate and detailed records of client care and services provided
Requirements:
- 0 years of experience in direct care or a related field (training will be provided)
- High school diploma or equivalent required
- Ability to lift and transfer clients with varying mobility needs
- Strong communication and interpersonal skills

- Reliable transportation and ability to travel to client homes
Work Environment:
- Work in clients' homes, with occasional visits to Upscale Home Care's office
