Requirements
- Recent Physical Examination
- MMRV Clearances (Measles, Mumps, Rubella, and Varicella titers or proof of immunization)
- Tuberculosis Screening (Negative QuantiFERON blood test or PPD skin test)
Qualifications
- Valid New York State LPN License
- Current CPR/BLS Certification
