Position Overview:
We are seeking an experienced Registered Nurse (RN) to join our growing team serving the Treasure Coast area. The ideal candidate will have psychiatric nursing experience and a passion for delivering quality home health care. You’ll be responsible for providing skilled nursing care, coordinating treatment plans, and supporting patients and families in managing their health at home.
Responsibilities:
Provide skilled nursing care to patients in accordance with physician orders and the established plan of care
Conduct comprehensive assessments and develop individualized care plans
Administer medications and treatments as prescribed
Monitor patient progress and communicate updates to the care team and physician
Educate patients and families on disease management, medication, and home safety
Document all patient interactions in compliance with Medicare and agency standards
Collaborate with interdisciplinary team members to ensure high-quality outcomes
Active Florida RN license (required)
1+ year of home health and psychiatric nursing experience
Current BLS certification
Strong clinical judgment, time management, and communication skills
Reliable transportation and willingness to travel within the Indian River territory
