Position Description
A direct care worker provides a number of services depending on the needs of the individual; these might include running errands, helping patients move around their homes, counseling individuals on independent living, and administering prescribed medication.
Duties:
Provide instruction and supervision of the following life skills within the home and/or community settings:
- Activities of daily living to include self-care and personal hygiene such as bathing, toileting and dressing using proper body mechanics
- Independent living skills such as housekeeping, meal preparation, preparing a budget and shopping
- Recreation and physical fitness activities (walking, swimming, fishing, bowling etc.)
- Communication skills including proper use of telephone and/or computer
- Intervene during aggressive or self-injurious behaviors
Implement Individual Service Plans (ISP):
- Practice Antecedent Management
- Engage in appropriate behavioral intervention and reinforcement
- Update log books including all information pertaining to behavioral, medical, skill building, and family and community interactions on a daily basis
- Communicate with regional center, day program staff, families and other service providers and agencies to include providing input for quarterly, semi-annual and annual meetings
- Assist in the development of social and recreational activities
We are looking for applicants that can work 4:00 p.m. to midnight (PM shift) or midnight to 8:00 a.m. (Graveyard shift) and can work weekends.
Successful candidates can successfully pass a Live Scan fingerprinting background screening and drug screen.
