Job description:
RN – Home Health Nurse Assessor | Per diem - 2 visit per week | North country-Essex NY
- Earn $55/hr + $90 Weekly Mileage Reimbursement
- North country - Essex/ Franklin /Clinton/Lewis
- Applicant can choose any one location
- Long-Term Contract | 12 months + Up to 5 Years Extension Potential

Your Role as an RN Nurse Assessor:
- Conduct in-home patient assessments (2 visit per week)
- Complete UAS-NY assessments accurately
- Evaluate medical, functional & care needs
- Coordinate with interdisciplinary healthcare teams
- Help patients access essential long-term care services
Requirements:
- Active New York RN License
- 2 Years of Experience in Home Health, Assessments, or Long-Term Care
- (either it is RN, LPN or HHA)
