Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using proper patient transfer methods such as Hoyer lifts and manual handling techniques.
- Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately to healthcare professionals.
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary, ensuring safety during meal times.
- Administer medications following care plans and HIPAA regulations, maintaining meticulous documentation of medication administration.
- Support clients with cognitive impairments such as dementia or Alzheimer’s by implementing memory care strategies and engaging them in meaningful activities.
- Provide transportation assistance for medical appointments, errands, or community outings to promote social engagement and independence.
- Maintain a clean, safe living environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces.
- Document patient observations, update care plans regularly, and communicate effectively with clients’ families and healthcare teams to ensure continuity of care.
- Assist with specialized care needs including hospice support, autism spectrum disorder support, or neurodevelopmental disorder management as required.
Skills
- Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments.
- Strong patient observation skills with the ability to recognize subtle changes in condition or behavior.
- Knowledge of dementia care, Alzheimer’s care, memory care, and hospice care best practices.
- Familiarity with patient monitoring devices such as blood pressure cuffs or pulse oximeters; ability to measure vital signs accurately.
- Experience administering medications safely according to prescribed care plans while adhering to HIPAA privacy standards.
- Ability to perform manual handling tasks safely, including heavy lifting and patient transfers using Hoyer lifts or other equipment.
- Excellent communication skills for effective client interaction and collaboration with healthcare teams and family members.
- First aid and CPR certification to respond swiftly during emergencies; knowledge of basic life support techniques.
- Experience working with individuals with disabilities or neurodevelopmental disorders such as autism or intellectual disabilities support needs.
- Compassionate caregiving attitude combined with strong organizational skills for managing multiple client needs efficiently.

Pay: $22.28 - $26.84 per hour
