Position Responsibilities
- Perform comprehensive in-home physical therapy evaluations and assessments
- Develop and implement individualized treatment plans
- Monitor and document patient progress, mobility, functional status, and outcomes
- Recommend therapeutic exercises, adaptive equipment, and home safety modifications
- Collaborate with physicians, nurses, occupational therapists, and other interdisciplinary team members
- Educate patients and caregivers on improving mobility, safety, and independence
- Maintain accurate and timely clinical documentation
Qualifications
- Current California Physical Therapist (PT) License in good standing
- Minimum of one (1) year of Physical Therapy experience
- Strong clinical assessment, communication, and documentation skills
- Current CPR certification
- Current TB clearance
- Ability to effectively manage a patient caseload and work independently in the field
Preferred Qualifications
- Home Health experience
- Hospice experience
- Experience working with geriatric and medically complex patient populations

Pay: $65.00 - $75.00 per hour
