Job Summary
With limited supervision from the Lead Service Coordinator, the Service Coordinator will be responsible for providing coordination services to adults with Developmental Disabilities (DD). Service Coordinator activities are designed to assist individuals in accessing needed social, educational, vocational, residential, and medical supports essential for living in the community in the most integrated and least restricted setting possible. The position will arrange, coordinate, monitor, evaluate, and advocate for services to meet each individual’s needs. The Service Coordinator will work with the individual, family, and other providers to find a person-centered solution for the individual supported.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Complete one in-person visit monthly to include required documentation.
- Assist individuals with goal setting, monitor/track progress as identified in the ISP, ensure relevance of goals and initiate IDTs to adjust goals as necessary.

Will add new outcomes/update existing outcomes in Therap and write accompanying teaching and support strategies.

Discontinue outcomes in Therap that are no longer applicable.

Responsible for updating demographic information in Therap.

Communicate with Case Managers to request ISPs/IDTs, discuss changes or concerns regarding budgets and services, gather and provide any requested documentation, etc.
- Responsible for updating the Tobosa Outlook calendar with ISPs, IDTs and other meetings for individuals.

Attend IDTs/ISPs for assigned individuals and complete/distribute meeting minutes to the appropriate department(s).

Complete responsibility sheets for ISPs with budget attached.

When prompted, complete a 48-hour review to ensure quality of ISP and budget.

Communicate with individuals, guardians, and IDT members to ensure continuity of services.

Assist in identifying resources to the individuals/teams.

Complete their own IST training, on all individuals prior to completing the PCA.
- Send Quality of Tool to all IDT members, family, and staff prior to completing PCA.

Complete PCA and Quarterly/Semi/Annuals in accordance with standards and state guidelines.

Ensure all services are being implemented as defined in the ISP.

Responsible for completing and returning all applications and recertifications for all public assistance/benefits.

Responsible for updating necessary agency spreadsheets.

Assist Team Coordinators with IST trainings, HCPs and MERPs, if designated, and only in case of an emergency.
- All other duties as assigned.
PROFESSIONAL REQUIREMENTS

High school diploma or higher education

Knowledge of various software programs such as: MS Word, MS Excel, and Outlook applications.

Must be able to multi-task demonstrating excellent organizational skills, writing skills, and problem-solving skills.

Must conduct self in a professional and approachable manner.

Excellent interpersonal skills and act as a team player.

Knowledge and experience with the Developmentally Disabled Waiver.
- Understanding of Developmentally Disabled Waiver budgets, residential services, outcome tracking, Developmentally Disabled Waiver Standards and NMAC Regulations, requirements for the Developmentally Disabled Waiver from the Department of Health preferred.
- Interest in and desire to work for non-profit organization that advocates for individuals with disabilities.

Flexibility in schedule and ability to work non-traditional hours.
- Must have reliable transportation.
- Able to pass a DOH background check and drug test.
