Greater Nashua Mental Health is seeking a Community Support Specialist to provide community-based support to individuals working toward recovery from mental health and co-occurring conditions. This role focuses on building engagement, supporting independence, and helping clients improve functioning within their natural environments through individualized, strengths-based interventions.
Key Responsibilities

Provide direct, community-based support services to individuals in their homes and community settings.

Engage clients using evidence-based interventions such as Motivational Interviewing.

Maintain a caseload and meet productivity expectations for direct client service hours.

Conduct outreach to re-engage clients who have disengaged from treatment.

Complete accurate and timely documentation for all services and outreach activities.

Collaborate with multidisciplinary teams to support coordinated, client-centered care.

Participate in team meetings, supervision, and ongoing training activities.

Maintain schedules and document services in the Electronic Medical Record (EMR).

Support clients in building life skills, accessing resources, and achieving recovery goals.

Complete mileage and expense reporting in a timely manner.

Perform additional duties or special projects as assigned.
Qualifications
-
Associate’s degree in Psychology or related field required; Bachelor’s degree preferred.

2+ years of experience in human services or behavioral health field.

Knowledge of mental health conditions and recovery-oriented practices.

Strong communication, interpersonal, and engagement skills.

Ability to work independently and collaboratively within a team environment.

Strong organizational and time management skills.

Proficiency with Microsoft Office and electronic documentation systems.

Ability to exercise sound judgment and maintain professional boundaries.
