About the Role:
As a Home Health Caregiver, you will play a vital role in delivering high-quality home health care services to our patients. Your primary responsibilities will include:
Providing personal care and assistance with daily living activities, such as bathing, dressing, and grooming
Assisting with medication management and administering medications as prescribed
Maintaining a safe and healthy environment for patients, including assisting with meal preparation and housekeeping tasks
Developing and implementing individualized care plans to meet the unique needs of each patient
Communicating effectively with patients, families, and healthcare providers to ensure coordination of care
Participating in ongoing education and training to stay current with industry best practices and standards
