The pay range offered will be based on a candidates years of experience, education and skills. We are seeking LVN's with 1 - 5 years experience as a LVN in primary care.

JOB SUMMARY:
This position provides safe patient care by assisting Clinic Providers with treatments, procedures and clerical duties as defined in the Licensed Vocational Nurse (LVN) Scope of Practice. This position will work in conjunction with Medical Providers to triage in person and telephone patients for the clinical site. In addition the LVN Nurse will work as the liaison for ER/Hospital discharge patients.
EDUCATION/QUALIFICATIONS:
- High school diploma or GED
- CA LVN license - Active
- BLS certification - Active
- Bilingual – Spanish and English required
- Minimum of one (1) year experience as LVN in a primary care medical clinic, community health center or FQHC. Preferably 1-5 years’ experience.
- Minimum of one (1) year experience doing triage strongly preferred
- Electronic Health Records experience highly desired, Epic preferred
RESPONSIBILITIES
- Manages the care of patients in collaboration with other health professionals and members of the health care team in order to make decision about treatment and attain health goals.
- Performs services requiring critical thinking as well as technical and manual skill. Performs basic nursing services, administers medication and communicates with patients with regard to the care plan and self-care requirements.
- Evaluates in person patients and telephone patients to assess patients clinically, dispense health advice, dispenses referral information, and schedule patients appropriately by indication as defined in the LVN Scope of Practice.
- Collaborates with local hospitals and patients to schedule primary care appointments for patients recently discharged from the hospital.
- Manages incoming phone calls, patient messages, and emails to ensure patients recently discharged from the hospital are contacted in a timely manner, according to SOP’s.
- Other duties as assigned by Nurse Manager.
- On Time arrival is critical responsibility to the position.

The starting pay range is based on meeting the minimum job requirements. Pay rate offered will be based on candidates experience, education and skill set.
