Schedule & Practice Details
- Monday–Friday schedule
- Mix of ESRD dialysis rounding and CKD office-based care
- Hospital rounding and inpatient nephrology management
- Travel between assigned clinics, dialysis centers, and hospitals required
- No prior nephrology experience required; training provided
Position Summary
The Advanced Practice Registered Nurse will work closely with nephrologists to provide comprehensive care for patients with chronic kidney disease (CKD), end-stage renal disease (ESRD), hypertension, and related conditions. The role includes outpatient clinic visits, dialysis rounding, hospital consultations, and care coordination across multiple healthcare settings.
Key Responsibilities
Outpatient Nephrology Clinic
- Evaluate, diagnose, and manage patients with CKD, hypertension, and related kidney disorders
- Develop and implement individualized treatment plans
- Order and interpret laboratory and diagnostic studies
- Educate patients and families regarding disease management and treatment options
- Coordinate specialty referrals and follow-up care
Dialysis Responsibilities
- Manage hemodialysis and peritoneal dialysis patients in accordance with established clinical guidelines
- Assess and manage complications associated with ESRD, including anemia, hypertension, mineral bone disease, electrolyte disorders, and vascular access concerns
- Conduct routine dialysis rounds and monitor treatment progress
- Collaborate with interdisciplinary care teams to optimize patient outcomes
- Support patients and families through treatment planning and ongoing education
Hospital Responsibilities
- Perform comprehensive patient assessments, histories, and physical examinations
- Participate in inpatient nephrology consultations and management
- Assist with admissions, discharge planning, and transitions of care
- Collaborate with physicians, nursing staff, case managers, and other healthcare professionals to ensure continuity of care
Qualifications
Required
- Master's or Doctoral degree from an accredited Nurse Practitioner program
- Current Florida APRN license or eligibility for licensure
- Hospitalist or inpatient practice experience required
- Experience managing hospitalized adult patients
- DEA registration or ability to obtain
- Strong clinical assessment, diagnostic, and patient management skills
- Excellent communication and interpersonal abilities
Preferred
- Nephrology, dialysis, internal medicine, hospital medicine, or critical care experience
- Experience caring for medically complex patient populations
- Familiarity with electronic medical record systems
