Position Summary
The Nurse Case Manager is responsible for monitoring and coordinating the healthcare needs of children in foster care. This role serves as a key liaison between medical providers, foster and biological families, and program staff to ensure children receive timely, comprehensive, and coordinated healthcare services.
The Nurse Case Manager provides clinical guidance, health education, case management, and advocacy while maintaining compliance with agency, state, and regulatory requirements.
Clinical Care Coordination
- Monitor and track the medical needs, treatment plans, and healthcare outcomes of children in foster care.
- Coordinate medical appointments, specialty care, and follow-up treatment with healthcare providers.
- Review physician recommendations and ensure implementation of prescribed treatment plans.
- Administer vaccines and provide nursing care within the scope of licensure.
- Document observations, interventions, physician orders, and care outcomes accurately and timely.
Family Support & Education
- Provide ongoing support, guidance, and health education to foster and adoptive parents.
- Communicate with foster families, biological families, guardians, and caregivers regarding medical conditions, medications, appointments, and treatment plans.
- Offer coaching and training to caregivers on managing children's health and medical needs.
- Advocate for children and families to ensure access to appropriate healthcare resources and services.
Program Collaboration
- Collaborate closely with case planners, social workers, medical providers, schools, and community partners.
- Participate in staff meetings, case conferences, foster care reviews, and multidisciplinary team meetings.
Compliance & Documentation
- Maintain accurate and up-to-date medical records and case documentation.
- Obtain and review health records, immunization records, birth records, newborn screening results, and other medical documentation.
- Ensure compliance with agency standards, regulatory requirements, and foster care health guidelines.
- Coordinate with child welfare agencies and external partners regarding medical and health-related matters.
Training & Education
Serve as a resource to staff and caregivers on child health and wellness issues.
Qualifications
Required
- Current New York State Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
- Current CPR Certification.
- Minimum of one year of nursing experience in a clinical, pediatric, community health, or related healthcare setting.
- Strong organizational, documentation, and case management skills.
- Excellent communication and relationship-building skills.
- Ability to work collaboratively with families, healthcare providers, and multidisciplinary teams.
Compensation
LPN: $30 – $35 per hour
RN: $75,000 – $92,000 annually
Additional differential for Spanish language skills

Pay: $63,700.00 - $92,000.00 per year
