Physical Therapist – Home Health

About the Position
We are seeking a compassionate, dependable, and experienced Physical Therapist (PT) to join our Home Health team. The Physical Therapist will provide skilled physical therapy services to patients in their homes, helping them improve mobility, strength, balance, functional independence, and overall quality of life.
This position is ideal for a PT who enjoys working one-on-one with patients and values providing personalized, patient-centered care in a home setting.
Responsibilities
- Perform comprehensive physical therapy evaluations and assessments in the patient's home.
- Develop individualized treatment plans based on the patient's diagnosis, condition, goals, and functional needs.
- Provide skilled physical therapy interventions to improve strength, mobility, balance, coordination, endurance, and functional independence.
- Educate patients and caregivers regarding therapeutic exercises, safety, mobility, fall prevention, and home exercise programs.
- Monitor patient progress and modify treatment plans as appropriate.
- Communicate with physicians and other members of the interdisciplinary healthcare team regarding patient status and progress.
- Complete accurate and timely clinical documentation in accordance with Medicare, agency, and regulatory requirements.
- Participate in care coordination and case conferences as needed.
- Maintain professional and compassionate relationships with patients, families, caregivers, and other healthcare professionals.
- Follow all applicable federal, state, and agency policies and regulations.
Qualifications
- Current and unrestricted Physical Therapist license in the State of California.
- Graduate of an accredited Physical Therapy program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and appropriate auto insurance.
- Previous home health experience is preferred but not required.
- Strong clinical assessment, communication, organization, and time-management skills.
- Ability to work independently and manage a flexible schedule.
- Comfortable traveling to patients' homes within the assigned service area

Pay: $100.00 - $160.00 per hour
