- Role: LPN
- Guaranteed Hours: 36 hours
- Contract Length: 13 weeks
- Shift: 7:00 AM – 7:30 PM
- Schedule: Weekend and holiday rotation required; No on-call
Requirements & Documentation
- Experience: 2+ years of LPN experience required; Doctor's office experience highly preferred.
- Licensure/Certifications: Active Illinois LPN license (via Nursys) and American Heart Association (AHA) BLS required.

- Skills checklist (pertinent to LPN, current to 1 year)
- Two completed supervisory reference forms with employment dates
- Documentation of permanent address (50-mile radius restriction applies)
Core Responsibilities
- Clinical Support: Assist providers with patient exams, treatments, and procedures.
- Communication: Manage patient phone calls, relay test results, and provide follow-up care instructions.
- Administration: Handle pre-certifications for tests/consults, submit prescriptions/refills, and maintain medical records.
- Lab Duties: Perform phlebotomy, specimen collection, and routine basic waived testing.
- Coordination & Education: Schedule tests/referrals and educate patients and families on care plans.
Pay: Up to $1,710.00 per week
