Summary
We are seeking a Private Duty Nurse to join our compassionate team at Chosen Village. This role involves delivering personalized nursing care to patients in their homes, ensuring their comfort and well-being while supporting their medical needs.
Responsibilities

Provide direct patient care, including monitoring vital signs and administering medications.

Assist with IV infusions and other medical procedures as needed.

Develop and implement individualized care plans based on patient assessments.

Educate patients and families on health management and medication administration.

Coordinate with healthcare professionals to ensure comprehensive patient care.

Support patients with dementia through tailored care strategies.
Requirements

Current nursing license in the state of Texas.

Proven experience in primary care or home health nursing.

Strong skills in patient assessment and medication administration.

Experience with IV infusion and dementia care preferred.

Excellent communication skills and ability to work independently.
