The Physical Therapist is a member of a multidisciplinary team at Expand Health at Home. The Physical Therapist performs physical therapy evaluations and treatments as ordered by qualified physicians in accordance with company standards and federal, state, and local standards, guidelines, and regulations.
The home health physical therapist’s goals are to rehabilitate and educate the patient to regain and retain their independence in their home setting. Further, the purpose of the physical therapist is to restore the patient’s function, alleviate pain, and prevent disabilities by planning and administering medically prescribed physical therapy.
- Assists the physician in evaluating the patient’s level of function.
- Assess patient’s home environment for safety hazards and architectural barriers, identify equipment needs.
- Identifies functional problems and establishes goals and a plan of care designed to improve the patient’s functional status (revising as necessary).
- Performs patient tests, measurements, and evaluations, such as range of motion and manual muscle tests, gait and functional analysis, and body parts measurement, and records and evaluates findings to aid in establishing or revising specifics of treatment programs.
- Performs and completes start-of-care, and ongoing assessments and associated paperwork in accordance to agency policy.
- Periodically re-evaluates and upgrades the patient’s program to maximize functional improvement.
- Administers the exercise program utilizing specialized therapeutic exercise techniques and equipment.
- Plans and administers prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury, or loss of body parts.
- Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
- Performs and submits required documentation of patient’s evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders.

Education and Experience: Must be a graduate from an accredited Physical Therapy program and licensed by the state of Ohio.
