A New York City nonprofit organization is seeking a Licensed Practical Nurse (LPN) for a promising Per Diem position with their Mobile Wellness Outreach team throughout the Bronx, Brooklyn, and Queens.
About the Opportunity:

Shift: Day
- Schedule: One weekday per week

Hours: 9am to 5pm (with a 1-hour unpaid lunch)

Setting: Field-based
Responsibilities:
-
Conduct health and functional assessments for clients in the community
-
Evaluate acute and chronic medical concerns and identify care gaps
-
Assess medication management needs and support adherence to care plans
-
Collaborate with the multidisciplinary team in case conferences, meetings, and planning sessions
-
Provide crisis support and document assessments, notes, and related tools in electronic systems
Qualifications:
-
At least 2 years of nursing experience
-
Completion of a Practical Nursing program
-
Current NYS Licensed Practical Nurse (LPN)
- Experience in Community-based, Mental Health, Behavioral Health, or Substance Use settings
-
Experience working with complex Medical and Behavioral Health conditions
-
Proficiency with Microsoft Office and electronic health records

Strong interpersonal and teamwork skills
