Job Overview
We are seeking a dynamic and experienced Full Time Home Health Field Nurse Supervisor to coordinate nursing and personal care services to patients at home. This vital role involves conducting home assessments, supervising nursing and paraprofessional staff, ensuring compliance with healthcare regulations, and delivering exceptional patient care. The ideal candidate will possess strong home care experience, extensive clinical expertise, and a passion for improving health outcomes in diverse environments.
ESSENTIAL JOB DUTIES:
- Conduct required in-home visits per physician orders and in accordance with regulations and agency policy.
- Assess patient needs, environment, and Plan of Care implementation during supervisory visits.
- Evaluate Nurses, Home Health Aide and personal care workers performance and competency in the home.
- Identify and address clinical concerns, care gaps, and safety issues; communicate findings to the care team.
- Provide on-the-spot coaching and training to field staff during visits.
- Document visits accurately and timely in the EMR.
- Collaborate with intake, scheduling, and operations on case starts, transitions, and care plan updates.
- Participate in on-call rotation as scheduled.
- Support the agency’s PI/CQI program through chart audits and process improvement activities as directed by leadership

Pay: $90,000.00 - $105,000.00 per year
