Every day, you’ll meet patients preparing to leave the hospital or an inpatient rehabilitation facility. Your role is to introduce Soteria-360, answer questions, and help eligible patients enroll in our Medicare-covered Transitional Care Program. You’ll be the face of Soteria-360 during one of the most important transitions in a patient’s healthcare journey.

About Soteria-360
The days and weeks following a hospital stay are among the most critical in a patient’s recovery. Our mission is to help patients stay safe at home by providing an additional layer of care and support during this vulnerable transition.
Soteria-360 is a Medicare-covered Transitional Care Program that works alongside a patient’s existing physicians and healthcare team—it never replaces them. Through physician oversight, telehealth, remote patient monitoring, and ongoing support, we help patients remain connected to their care while reducing avoidable hospital readmissions.
What You’ll Do
Build Trusted Relationships with Healthcare Professionals
- Develop strong relationships and become a trusted partner with physicians, nurses, case managers, discharge planners, and rehabilitation staff within partner facilities
- Collaborate with clinical teams to identify appropriate patients
- Maintain and strengthen professional relationships throughout your territory
Meet Patients Before Discharge
- Visit patients in hospitals and inpatient rehabilitation facilities
- Introduce Soteria-360 and explain how our Medicare-covered Transitional Care Program provides additional support after patients return home.
- Answer questions and address concerns
- Help eligible patients enroll in the program
- Complete enrollment paperwork and required consents
Support Patients After Discharge
After enrollment, you’ll help ensure patients get off to a successful start in the program. Depending on business needs and territory coverage, you may also:
- Visit patients in their homes following discharge
- Deliver and set up simple remote monitoring equipment
- Connect devices to the patient’s home Wi-Fi, when needed
- Verify equipment is functioning properly
- Teach patients and caregivers how to confidently use the equipment
As our team grows, some in-home installations may be completed by dedicated team members, allowing you to focus more on patient enrollment and relationship building.
No advanced technical experience is required. We provide training. Candidates simply need to be comfortable using tablets, smartphones, Wi-Fi, and connected devices.

Qualifications
Required
- Excellent communication and interpersonal skills
- Comfortable speaking with patients, families, and healthcare professionals in a variety of settings.
- Ability to build trust and rapport quickly
- Confidence initiating conversations with patients and healthcare professionals
- Professional appearance and demeanor
- Comfortable working independently in a field-based role
- Reliable transportation and willingness to travel throughout the territory
- Strong organizational and follow-through skills
- Comfortable using basic technology and connected devices
Preferred
- Experience working with seniors
- Experience enrolling patients into healthcare programs
- Experience discussing healthcare services with patients and families
- Consultative sales or relationship-building experience
- Hospital, rehabilitation, hospice, home health, physician office, or healthcare outreach experience
- Bilingual English/Spanish
Schedule
- Part-time (approximately 20–25 hours per week)
- Primarily weekday daytime hours
