Position Overview

What You’ll Do
- Evaluate and treat SNF residents as a Physical Therapist with post-acute, long-term care and rehab needs
- Design individualized, evidence-based treatment plans that prioritize function and quality of life
- Complete clear, timely documentation
- Partner closely with nursing, physicians, families and an interdisciplinary rehab team
- Help plan, market and develop new programs that benefit residents and the facility
- Provide in-services and education to staff
- Support the smooth, day-to-day operation of the rehab department
What We’re Looking For
- Bachelor’s, Master’s or Doctorate in Physical Therapy
- Graduate of an accredited physical therapy program
- Possess or be eligible for Minnesota PT licensure
