Field Licensed Practical Nurse (LPN) – Home Health
Territory: East Oklahoma (Choctaw, Midwest City, Shawnee)
Compensation: $30–$35 per visit
Schedule: Monday-Friday, Full-Time
Benefits: Full benefits package + gas mileage reimbursement
About the Role
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team in West & North OKC. The ideal candidate will have at least 3 years of home health nursing experience and a passion for providing high-quality, patient-centered care in the home setting. This role offers flexibility, autonomy, and the opportunity to build meaningful relationships with patients and families.
Qualifications:
- Current Licensed Practical Nurse (LPN) license in the state of Oklahoma.
- Minimum of 3 years of nursing experience (home health experience required).
- Strong clinical assessment and documentation skills.
- Reliable transportation and a valid driver’s license.
- Compassionate, dependable, and committed to providing exceptional patient care.
Responsibilities:
- Provide direct nursing care to patients in their homes under the supervision of an RN and in accordance with the plan of care.
- Conduct patient assessments, monitor vital signs, and accurately document progress.
- Administer medications and treatments as prescribed.
- Educate patients and families on care plans, medications, and health maintenance.
- Communicate changes in patient condition promptly to the supervising RN and physician.
- Ensure compliance with agency policies, procedures, and state/federal regulations.
- Travel to patient homes within Central OKC (mileage reimbursement provided).

Pay: $60,000.00 - $70,000.00 per year
