We are looking for an LVN to work as a liaison to visit our Home Health patients in Los Angles County (West LA to South LA) to ensure our care is upstanding and that the service line is appropriately meeting their needs.
Requirements:
- Active LVN license
- Home Health and Hospice experience
- Professional demeanor
- Willingness to visit up to 5 patients daily.
This position is strictly a liaison role.
Clinical knowledge will only be extended through conversation with patients and family members of patients and then shared with the clinical management team at the office.

Pay: $75,000.00 - $90,000.00 per year
