Job Overview:
The Sycamore of River Falls is a growing, we are a full-service senior living community with 84 apartments including 68 blended independent and assisted living and 16 memory care apartments in a secured neighborhood on first floor. The property has four villa patio twin home units with two bedroom and three bedroom options for active, independent seniors. The apartment building consists of a variety of studios, one-bedroom, one-bedroom plus den, and two-bedroom units. The underground parking garage has 50 parking stalls, maintenance rooms, and residents’ storage area. Managed by Ebenezer. This newer care facility is Located in River Falls, WI
This position is responsible for ensuring that the safety and health needs of the residents are met.
Responsibilities

Maintains current assessment and documentation of resident health status

Performs initial and on-going assessments per the applicable Wisconsin home care regulations and Nurse Practice Act. Ensures RN Assessment is completed at least once per year and when there is a change in health condition.

Works with resident and/or responsible party through collaboration, education and timely communication to ensure appropriate service and placement.

Assures that residents have up-to-date service agreements and care plans. Implements service agreements with new residents. Reviews and updates service agreements at least once per year and more frequently when there is a change in service needs.

Develops, maintains, and implements individual emergency plans based on health status and Advance Care Planning documents, including POLST.

Communicates changes in resident conditions to medical and other relevant individuals.

Assures that all physician orders are signed and implemented per the applicable Wisconsin home care regulations.

Manages and oversees medication and treatment administration.

Assures accuracy of medication and treatment records, including documentation of administration and appropriate signatures.

Monitors work and performance of licensed and unlicensed staff.

Meets with staff to coordinate care and address care issues.

Assists Director of Health Services (DHS) in monitoring and evaluation of staff performance.

Schedules and completes Supervisory Visits.
Required Qualifications

Associates Degree in Nursing

1 year related experience

Proficient in Microsoft Windows Applications

WI Registered Nurse (RN) License

Basic Life Support (American Heart Assoc or Red Cross) Upon Hire
Preferred Qualifications

Bachelors Degree in Nursing

Proficient in Eldermark Computer Documentation Software

Prior supervisory experience

Prior experience in home care or assisted living environment
Qualifications:
$37.67- $54.42 Hourly
