Job Overview
We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will have a strong background in home health care. This role involves working with diverse patient populations, including geriatrics, orthopedics and neuro, to develop and implement effective treatment plans that enhance mobility, relieve pain, and improve overall physical function.
Responsibilities
- Conduct thorough evaluations of patients' physical abilities and limitations.
- Develop individualized treatment plans based on patient assessments and goals.
- Implement therapeutic exercises and interventions to improve strength, flexibility, and coordination.
- Educate patients and their families on treatment protocols and home care strategies.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Maintain accurate documentation of patient progress and treatment outcomes.
- Stay updated on the latest advancements in physical therapy practices and techniques.
Requirements
- Valid state license to practice as a Physical Therapist.
- Strong knowledge of anatomy and physiology relevant to physical therapy.
- Experience in geriatrics, orthopedic and home community care is preferred.
- Familiarity with kinnser system for patient management and documentation.
- Excellent communication skills for effective interaction with patients and healthcare teams.
- Ability to work independently as well as part of a collaborative team environment.
- Commitment to ongoing professional development and continuing education in the field.

Job Types: PRN, Contract, Per diem
Pay: $65.00 - $90.00 per hour
Expected hours: 1 – 30 per week
