Responsibilities
- Assess patient health status through comprehensive evaluations, including vital signs, medical history, and physiological assessments related to dialysis treatment.
- Educate patients and their families on peritoneal dialysis procedures, infection prevention, and self-care techniques to promote independence and safety at home.
- Monitor and manage patients’ treatment plans by reviewing laboratory results, managing complications, and adjusting therapy as needed in accordance with healthcare policies.
- Coordinate discharge planning by preparing patients for outpatient dialysis sessions, scheduling follow-up appointments, and ensuring proper documentation in electronic health records (EHR).
- Conduct routine patient assessments during visits or home visits to evaluate treatment effectiveness and identify potential issues such as infections or fluid imbalances.
- Collaborate with physicians, case managers, and other healthcare professionals to develop individualized care plans that meet regulatory standards and optimize patient outcomes.
- Maintain compliance with HIPAA regulations and hospital policies while managing sensitive medical records and ensuring confidentiality at all times.
Requirements
- Valid nursing license with current registration in the relevant state or region.
- Critical care experience highly preferred, especially within ICU or emergency room settings; Level I Trauma Center experience is a plus.
- Demonstrated knowledge of physiology, medical terminology, infection control practices, and hospital medicine protocols related to renal care.
- Experience with electronic health records (EHR) management and medical records documentation.
- Strong patient service skills with the ability to perform thorough patient assessments and discharge planning effectively.
- Familiarity with healthcare regulations including HIPAA compliance and health policy standards.
- Previous outpatient or clinic experience in nephrology or dialysis settings is desirable; inpatient or post-acute care experience is beneficial.
- Ability to manage multiple priorities efficiently while maintaining high standards of patient care in a fast-paced environment.
