The Health Information Specialist is responsible for accurate scanning, indexing, maintenance, and release of patient medical records in accordance with hospital policy and state and federal privacy regulations. This position ensures timely, complete, and secure management of health information to support patient care, continuity of services, legal compliance, and authorized information requests.

High School Diploma or GED required

Previous medical records, health information, or medical office experience preferred

Knowledge of medical terminology preferred

Knowledge of HIPAA and patient privacy regulations preferred

RESPONSIBILITIES:

Scans, indexes, and files medical records into the electronic medical record (EMR) accurately and in a timely manner.

Reviews incoming documents to ensure proper patient identification, completeness, and correct document type prior to scanning.

Processes medical record release of information (ROI) requests in compliance with HIPAA, state law, and organizational policy.

Verifies authorization forms and patient identity prior to releasing protected health information.

Maintains confidentiality and security of all medical records and patient information at all times.

Tracks, logs, and documents all requests for records and releases according to department procedures.

Responds to requests from patients, providers, attorneys, insurance companies, and other authorized parties within established timeframes.

Performs quality checks to ensure scanned documents are legible, correctly indexed, and properly routed in the EMR.

Assists with record corrections, amendments, and documentation reconciliation as needed.

Communicates professionally and courteously with patients, staff, and external contacts regarding medical record requests.

Maintains organized electronic and physical filing systems in accordance with retention and destruction policies.

Supports clinic operations by assisting with audits, chart reviews, and compliance activities as requested.

Maintains professionalism, confidentiality, neat appearance, and a positive, service-oriented attitude.
