Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting to ensure comfort and dignity.
- Monitor patients’ health status by observing vital signs such as blood pressure, pulse, temperature, and respiration; report any concerns promptly.
- Support medication administration following care plans and HIPAA regulations to maintain safety and confidentiality.
- Prepare nutritious meals aligned with dietary needs and assist with meal planning and cooking to promote healthy living.
- Maintain a clean and safe environment by performing light cleaning duties and ensuring proper sanitation procedures are followed.
- Use specialized equipment such as Hoyer lifts or resident’s <PERSON> to assist with mobility and prevent injury during transfers.
- Document care activities accurately in compliance with facility protocols and state regulations; update care plans regularly.
- Provide emotional support through active listening, companionship, and engaging in social activities to foster mental well-being.
- Support residents with memory care needs related to dementia or Alzheimer’s by implementing personalized strategies for safety and engagement.
- Collaborate with healthcare professionals, social workers, and family members to deliver comprehensive patient-centered care.
Experience
- Prior experience working in assisted living, nursing homes, or group homes is highly preferred.
- Demonstrated knowledge of patient observation techniques, vital signs monitoring, and medication administration.
- Experience providing dementia or Alzheimer’s care, hospice support, or working with individuals with developmental disabilities is advantageous.
- Familiarity with HIPAA regulations to ensure patient confidentiality at all times.
- Hands-on experience using mobility aids such as Hoyer lifts or resident lifts for safe transfers.
- Strong background in home & community care environments or home health services is desirable.
- Ability to perform heavy lifting safely when assisting residents with mobility needs.
- Certification in CPR and First Aid is required; additional certifications like group home experience or nursing assistant credentials are a plus.
