We are seeking a motivated and compassionate Admissions Coordinator to join our skilled nursing team. This position plays a key role in healthcare admissions, census development, referral marketing, and resident transitions.
The Admissions Coordinator works closely with hospitals, discharge planners, case managers, physicians, residents, and families to coordinate a smooth and successful admission process.

Manage the skilled nursing admissions process from initial inquiry through admission
Screen prospective residents and determine eligibility based on facility admission criteria
Review clinical and medical information to assist with admission decisions
Obtain and verify insurance, financial, and resident information
Coordinate admissions with hospitals, discharge planners, case managers, physicians, families, and referral sources
Conduct facility tours and answer questions from prospective residents and families
Build and maintain strong relationships with hospital and community referral sources
Make regular phone, email, and in-person contacts with referral partners to support census growth
Develop and maintain referral relationships within the local healthcare community
Follow up on inquiries and referral opportunities
Coordinate community and referral-source events
Ensure admission paperwork and documentation are complete and accurate
Help residents and families navigate the transition into the nursing center
Address resident and family concerns and help resolve issues during the admission process
Participate in census and admissions meetings
Maintain knowledge of the local market, referral sources, and resident demographics
Monitor admissions trends and prepare reports as needed
Work collaboratively with nursing, social services, administration, and other departments
Participate in after-hours and weekend admissions/tours when needed
Previous experience in healthcare admissions, healthcare sales, marketing, long-term care, skilled nursing, senior living, or a related field preferred
Sales, customer service, or relationship-building experience strongly preferred
Knowledge of the skilled nursing or healthcare industry is a plus
Strong communication, organization, and follow-up skills
Comfortable speaking with residents, families, hospital staff, and healthcare professionals
Professional appearance and communication style
Ability to work independently while collaborating effectively with a team
Strong attention to detail and ability to manage multiple priorities
College degree preferred, but relevant experience will be considered
