Granite Creek Health and Rehabilitation, is seeking a Physical Therapist (PT) to join our Outpatient Therapy team! Responsibilities
- Provide patient care through physical therapy techniques, utilizing knowledge of physiology, anatomy, and medical terminology.
- Develop and implement care plans, including discharge planning and acute care for patients of all ages.
- Collaborate with healthcare professionals to deliver quality care in various settings such as inpatient, outpatient, and home health.
- Utilize premium skills in pediatrics, trigger point therapy, orthopedics, and geriatrics to address diverse patient needs.
- Document patient progress accurately in EMR systems to ensure continuity of care.
Qualifications
- Patient care experience
- Proficiency in medical terminology and anatomy
- Knowledge of physiology and physical therapy techniques
- Experience in discharge planning and care plans
- Familiarity with acute care settings and pediatric patients
- Ability to work with EMR systems and provide home & community care
- Previous experience in inpatient, outpatient, and clinic settings
- Certification in trigger point therapy and experience in orthopedics, geriatrics
- Strong communication and interpersonal skills
- Ability to collaborate with a multidisciplinary team
