Gilead Health Care is seeking full-time or PRN skilled nurses to assist patients in the Central / South Austin area.
While Gilead is based in Round Rock, there is a need for Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) to provide care to patients in Central / South Austin and the surrounding regions.
** Pay based on experience**
Responsibilities:
- Provide skilled nursing care to patients in their homes or nursing facilities
- Conduct comprehensive assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans as needed
- Educate patients and their families on managing their health conditions
- Collaborate with other healthcare professionals to coordinate patient care
- Maintain accurate and up-to-date patient records
Qualifications:
- Valid Registered Nurse (RN) license in the state of practice - Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Minimum of 1 year of nursing experience, preferably in home health or a related field
- Implement the individualized POC and recommend revisions to the plan as necessary.
- Knowledge of health coaching principles and techniques
- Familiarity with durable medical equipment (DME) and medical imaging procedures
- Proficiency in using electronic health record systems, such as Wellsky/ Kinnser
- Strong communication and interpersonal skills
