Wayne County Health Department is seeking a full-time Child and Family Connections Service Coordinator for the Illinois Early Intervention Program. The candidate must have a bachelor’s degree or higher in Human Services or related field or hold a current license as a Registered Nurse. The applicant must be proficient in computer skills, organized, possess good communication skills, multi-tasking skills, and be willing to travel for home visits.

Pay: $42,000.00 per year

Job Requirements:
- Receive referrals; develop, maintain, and process the permanent Early Intervention case record
- Facilitate and participate in developing the Individual Family Services Plan (IFSP) to support the child and assist the family in monitoring IFSP implementation and progress
- Work collaboratively as a team with family and service providers to support children enrolled in the EI Program.
- Coordinate and ensure completion of initial and annual evaluations and assessments
- Provide ongoing advocacy support to ensure that appropriate EI services
- Maintain accurate documentation in the form of case notes
- Facilitate the development of a transition plan to preschool, school, or other services
Travel requirements:
- CFC#23 serves seven counties (Wayne, Edwards, Wabash, Hamilton, White, Gallatin, and Saline)
