Job Overview
We are seeking a dedicated and compassionate RN - Certified Case Manager to join our healthcare team. The ideal candidate will have experience in clinic settings, primary care, or infectious disease care, and will be responsible for coordinating patient care across various healthcare services. This role is crucial in ensuring that patients receive comprehensive and effective treatment while navigating their healthcare journey.
Responsibilities
- Assess patient needs and develop individualized care plans in collaboration with multidisciplinary teams.
- Coordinate outpatient services and facilitate access to necessary resources for patients.
- Conduct discharge planning to ensure smooth transitions from hospital to home or other care settings.
- Monitor patient progress and adjust care plans as needed based on ongoing assessments.
- Provide education and support to patients and their families regarding treatment options and health management.
- Document all interactions and updates in the patient's medical records accurately.
- Collaborate with healthcare providers, including those in geriatrics, emergency room (ER), and outpatient settings, to ensure continuity of care.
- Perform vital signs checks as part of routine assessments when necessary.
Qualifications
- Previous experience in a clinical setting, preferably within primary care or infectious disease care.
- Knowledge of discharge planning processes and outpatient service coordination.
- Strong communication skills with the ability to work effectively with patients, families, and healthcare professionals.
- Familiarity with geriatric care principles is a plus.
- Ability to manage multiple cases efficiently while maintaining attention to detail.
- A compassionate approach towards patient care with a focus on improving health outcomes.
- Certification or licensure in nursing or social work may be preferred but not required.
