Position Summary
The Host Home Clinical Coordinator oversees host home providers and the individuals receiving services. This role is responsible for ensuring high-quality, compliant, and person-centered service delivery by monitoring provider performance, supporting client needs, and coordinating services in alignment with individualized service plans and regulatory requirements. The Host Home Clinical Coordinator serves as a primary regional point of contact for host home providers, clients, guardians, and internal leadership.

Essential Duties & Responsibilities Host Home Provider Oversight
- Serve as the primary field contact for assigned host home providers.
- Oversee and support host home providers within the assigned region
- Serve as the primary contact for host home providers regarding program expectations, service delivery, and compliance
- Conduct regular home visits, monitoring contacts, and provider check-ins
- Review provider documentation for accuracy, completeness, and compliance
- Provide guidance, coaching, and corrective direction to host home providers as needed
- Identify concerns related to provider performance, safety, or compliance and escalate appropriately
Client Oversight & Support
- Oversee services for clients residing in host homes within the region
- Ensure services align with individualized service plans, assessed needs, and authorizations
- Participate in placement compatibility oversight during trial periods.
- Monitor client health, safety, progress, and overall well-being
- Coordinate care with guardians, case managers, and interdisciplinary team members; respond to changes in needs, incidents, or concern
- Advocate for client rights, preferences, and person-centered outcomes
Program Coordination & Monitoring
- Coordinate and monitor regional service delivery to ensure consistency and quality
- Identify service gaps, risks, or trends and implement corrective or preventive actions
- Participate in interdisciplinary team meetings, planning meetings, and reviews
- Communicate regional updates, concerns, and outcomes to leadership
Referral & Intake Support
- Assist with the referral-to-intake process for prospective clients
- Receive and help gather initial referral information from MCOs, case managers, families, and other sources
- Support the Director of Operations with intake preparation, including collection and organization of required documentation
- Participate in preliminary reviews of client needs, eligibility, and service requirement
- Coordinate communication between referral sources, leadership, and regional staff throughout the intake process
- Ensure referrals progress timely and accurately from intake to placement consideration
Documentation & Compliance
- Complete timely, accurate, and thorough documentation, including case notes, progress summaries, and required reports
- Ensure compliance with state, federal, and agency regulations, including HCBS and Medicaid requirements
- Maintain organized and audit-ready regional and individual records
- Ensure confidentiality and HIPAA compliance at all times
Health, Safety & Incident Response
- Monitor and address health and safety concerns involving clients or host homes
- Respond to incidents, allegations, or emergencies in accordance with agency policy
- Support investigations and follow-up actions as required
- Ensure corrective actions are implemented and documented
Administrative & Quality Responsibilities
- Track required contacts, visits, deadlines, and follow-ups for providers and clients
- Assist with audits, licensing reviews, and quality assurance activities
- Participate in trainings, supervision, and leadership meetings
Qualifications Required
- Bachelor’s degree in Human Services, Social Work, Psychology, or a related field (or equivalent experience, if permitted)
- Experience coordinating services in community-based or residential settings
- Strong clinical judgment and independent decision-making ability
- Experience working with individuals with ID/DD and/or mental health diagnoses
- Strong organizational, documentation, and time-management skills
- Ability to manage multiple host home providers and clients independently
- Strong communication, professional judgment, and problem-solving skills
- Valid driver’s license and reliable transportation
Preferred
- Experience conducting in-home assessments or safety evaluations
- Knowledge of Iowa HCBS waivers and Medicaid-funded services
- Experience with electronic health records or case management systems
- Experience navigating crisis situations
Work Environment & Expectations
- Combination of office, community, and in-home work
- Regular travel within assigned region
- Flexible scheduling may be required to meet provider and client needs
- On-call or after-hours responsibilities may be required depending on program needs

Pay: $24.98 - $27.50 per hour
